This includes four documents: 1) Fulfillment of Basic Requirements Form, 2) Release of Liability Form #1, 3) Release of Liability Form #2 and 4) Preliminary Application Form..

Print all pages of each document and complete in pen (do not use a typewriter). Submit the forms either in person at or by mail to the Gibson County Sheriff's Department, 112 E.
